Eligibility Criteria and Referral Information
Our interdisciplinary teams provide care for vulnerable community members of all ages and in all boroughs of New York City, regardless of their ability to pay.
The programs below require a referral. We also offer programs that your patients, clients, and their family members can access directly without a referral. Our comprehensive list of programs and services includes eligibility requirements and can be filtered by age, location, and need.

Behavioral Health Post-Acute Transitional Care
VNS Health works in close collaboration with health plan and provider teams to provide post-acute transitional care to individuals with chronic and complex behavioral health conditions.
Our early intervention solutions are designed to interrupt the cycle of inpatient and emergency department admissions and readmissions and improve overall health outcomes.
We deliver comprehensive, interdisciplinary care management and treatment, at increasing levels of intensity, deploying integrated, needs-based clinical support and resources — at home and in the community.
