VNS Health offers specialized behavioral health services designed to support individuals with mental health or substance use disorders. If someone you know needs additional support, you can refer them to one of our community programs.
VNS Health can provide support to those living with:
- Addiction and substance use disorders
- Alzheimer’s disease or another type of dementia
- Anxiety disorders
- Bipolar disorder
- Major depressive disorders
- Schizophrenia
- Other psychiatric diagnoses
Community Programs and Eligibility Requirements
VNS Health offers many community programs that might benefit your patients. Each program serves different people and different areas of New York. For information about these programs or to refer someone, please complete the Behavioral Health Referral Form.

Behavioral Health Support at Home
If you are a social worker, community partner, or social service partner, you might notice harmful behaviors in your clients. VNS Health’s behavioral health services can help individuals in all aspects of their lives — whether that’s building relationships, achieving their dreams, or completing daily tasks.
In addition to our community programs, VNS Health supports individuals with various home care plans. When needed, a behavioral health specialist is available to support home care nurses as a part of an individual’s overall care plan. This can be beneficial in helping someone address all aspects of their health — including physical, mental, and emotional health.
