Adult Mobile Crisis Teams

Rapid, in-home mental health assessment and linkage to services for adults who are experiencing—or are at risk of experiencing—a mental health crisis.

Article 31 Outpatient Clinic and School-Based Clinics at PS 369 & MS 179

Team of social workers and psychiatrists/nurse practitioners who can help seriously emotionally and behaviorally disturbed children, teens, and their families with assessment, therapy, and medication management.

Assertive Community Treatment (ACT)

Long-term outpatient psychiatric care, medication management, medical support, care coordination, and case management for seriously mentally ill adults with multiple psychiatric hospitalizations and a lack of engagement with traditional outpatient services.

Behavioral Health Community Transitions

Short-term bridge to care services to assist with transition from inpatient units to the community for adults and children who have a diagnosed mental illness and are currently in or were recently discharged from an inpatient psychiatric unit.

Children’s Health Home

Comprehensive care coordination that integrates medical and behavioral health services.

FRIENDS Program (Children & Family Support Team and STEPS)

Training in helpful mental health skills and linkages to community resources. Offers support two times a week for 6–8 weeks — with follow-up support for up to 4 months.

Health Home Care Management

Long-term care coordination and support for adults with behavioral health or substance use problems, HIV/AIDS, or multiple chronic illnesses.

Home-Based Crisis Intervention

Short-term, in-home services (crisis intervention, short-term case management, family support) for families with children who are at risk of hospitalization due to serious mental health conditions.

Intensive Mobile Treatment (IMT)

Long-term outpatient psychiatric care, medication management, medical support, care coordination, and case management for seriously mentally ill adults with a recent history of frequent hospitalization, housing instability, and incarcerations.

Parachute

In-home counseling, medication management, crisis intervention, and care management for adults who are high utilizers of emergency departments with a history of poor engagement in their current behavioral health treatment options.

Pathway Home

High-touch critical time intervention and care coordination for high utilizers of Harlem Hospital psychiatric inpatient and emergency departments.
