Health care can be complicated, and VNS Health is here to make referring a patient to the services they need as easy as possible. When you refer patients to VNS Health, you can know they are in the hands of expert and compassionate caregivers.

Q: What types of services does the VNS Health Home Care team provide?

A: For patients who need acute care (typically after a hospital or nursing home discharge or after a visit with their primary care physician), VNS Health is proud to offer:

Q: What insurances does VNS Health accept?

A: VNS Health accepts a wide variety of insurances as well as most insurances based in New York State. For a comprehensive list of insurances we are contracted with, please contact us.

Q: Will my patient’s insurance cover home care, and are there any copays?

A: Because there are multiple products within any insurance plan, a patient may or may not have copays. Our insurance department will conduct a thorough review to evaluate whether the patient has any copays.

 

Q: How do I make a referral?

You can refer patients to VNS Health services online or by phone or fax.

Q: What is needed at the time of referral?

A: At a minimum, VNS Health’s intake team will need: 

  • Patient’s name
  • Patient’s address
  • Patient’s date of birth
  • A general explanation of what services the patient may benefit from

For the patient’s services to begin, our intake team will need orders from a physician or qualified practitioner (NP or PA). 

Q: Are all field clinicians vaccinated?

A: Yes. New York State required all individuals providing care in the home — including home health aides — to be vaccinated against COVID-19 by October 7, 2021. Aides can request an exemption for medical or religious reasons. Because of strict privacy rules, we cannot share the vaccination status of any of our employees. Patients may talk to their aide directly if they have any concerns.

Q: Do you accept COVID-positive patients?

A: Yes, our VNS Health field clinicians are fully equipped with the proper protective personal equipment and protocols to provide home care to COVID-positive patients. 

If a patient was COVID-positive within the past 14 days but is no longer positive, it would be ideal to provide proof of a negative test result.

Q: How many hours of home health aide services do you provide?

A: When a patient’s acute care services begin at VNS Health, our field clinician will conduct a home health assessment to evaluate whether the patient is eligible for services and to estimate the number of home health aide hours the patient may qualify for. The number of hours a patient may qualify for is also contingent on the their insurance.

Q: What if my patient or their caregivers need extra help?

A: VNS Health also offers private-pay services should a patient request additional hours not covered by their insurance. VNS Health offers private personal care services and geriatric care management.
