Our Chronic Condition Management program gives you extra care and support to help you manage your health in the comfort of your home. Provided by our Medical Care at Home team, we’ll work with you, your doctor, and your other care providers to help keep you safe and out of the hospital. You’ll get care through visits, phone calls, and video chats.
This specialized support is in addition to the care you already receive — and comes at no or low cost to you.

Extra Support Personalized for Your Needs
We know each individual’s needs and health goals are different. As part of this program, you’ll have a care plan designed just for you.
Here are some examples of how your care plan can support you:
- Learn to manage your health: Get easy tips for staying healthy and using your medicines the right way.
- Get help with scheduling and supplies: We’ll help you set up doctor visits and make sure you have needed medications and supplies.
- Stay safe at home: Receive health checks at home — like checking blood pressure or blood sugar — and screenings to help prevent falls.
- Use tools to track your health: Simple devices at home can watch for changes in your health and alert our care team if something is wrong.
- Talk to a nurse anytime: Call our care line 24/7, even on nights and weekends, if you have questions or don’t feel well.
How the Program Works
Here’s what you can expect as part of the Chronic Condition Management program:
- Start with a VNS Health visit: A VNS Health nurse practitioner will meet with you to learn about your health, home safety, and social needs.
- Get a care plan made just for you: A nurse care manager will create a plan based on your health goals and what matters most to you.
- Have a dedicated care team to support you: Your team may include a nurse care manager, nurse practitioner, social worker, and care coordinator.
- Stay connected: We’ll check in with you regularly to see how you’re doing and update your care plan if needed.
- Work together with your doctors and caregivers: We’ll share important health updates with your doctors, other care providers, and family members to keep everyone on the same page.
