VNS Health Achieves CHAP Age-Friendly Care Certification and Deemed Status

In late October, after several days of onsite visits, inspectors from CHAP (Community Health Accreditation Partner) renewed accreditation for VNS Health Home Care. In addition, for the first time ever, CHAP awarded the Home Care team Age-Friendly Care at Home certification and Deemed Status.

The CHAP inspectors also praised the quality of care that our clinicians are providing to patients and commended the compassion and dedication of the entire Home Care team.

Why it matters: Age-Friendly Care certification affirms that our Home Care’s clinicians are consistently utilizing the Age-Friendly Care “4-Ms” protocol with patients. This evidence-based, patient-centered care delivery model has been shown to improve health outcomes and patient satisfaction levels.

In addition, CHAP awards Deemed Status to home care providers who display a consistently high quality of care, based on a rigorous review of operations. The designation highlights the overall excellence of our Home Care’s clinical and support operations and also exempts the organization from separate tri-annual inspections by New York State’s Department of Health.
